If we accept your insurance, we must verify your benefits and call you back with all the details. Keep in mind, most insurance plans do require a recent pediatrician referral stating “ST Eval & Treatment” or “OT Eval & Treatment”; and some require a prior authorization to be completed by your pediatrician’s office.

If we are not providers with your insurance, we do offer affordable out-of-pocket rates.

In all cases, we require a copy of a guardian’s driver’s license and complete case history form. Please download the file in the language most comfortable and send that over to us at
